This is the "Identity Theft Victims' Rights Act of 2004." It makes it illegal to knowingly obtain, possess, buy, or use another person's personal identifying information (like their name, Social Security number, etc.) without their permission with intent to commit fraud, obtain credit or services in their name, or illegally obtain prescription medications. The base punishment is a Class A misdemeanor (up to 11 months 29 days jail and $2,500 fine), but it can be enhanced to Class C or D felonies for repeat offenses or aggravating factors.

Violations – Penalty – Venue – Providing records for prosecution — Class E felony. Up to 1-6 years in jail and $3,000 fine. Penalty may be enhanced for prior offenses or aggravating factors.

It is illegal to knowingly fail to appear in court when you've been summoned, arrested with a court-ordered release condition, cited instead of arrested, or issued a criminal summons. Failure to appear is a Class A misdemeanor punishable by up to 11 months 29 days in jail and a $2,500 fine. A defense exists if you had a reasonable excuse for missing the court date.
